• Is the coding accurate?

• Does the documentation support the codes used and establish medical necessity?

• Have NCCI edits been checked?

• Have all LCDs and NCDs been followed?

• Has the payer-specific medical policy for the procedure been reviewed and adhered to?

• Has accurate prior authorization been obtained?

• Has the authorization been updated if anything changed during the procedure?
